Targeted Case Management

Provide case management to children adults, and families struggling with mental health illness and/or substance abuse to assist them in gaining access to necessary services such as: medical, social, educational, and vocational.

 

Targeted Case Manager will assist individuals with community resources, application processes, obtaining documentation, medical appointments, transportation, job search, and assisting with facilitating treatment options.
